Trace the decision path

A reviewable cognitive workflow connects intent, context, evidence, constraints, proposed decisions, approvals, actions, and outcome signals. The trace should make the source and status of each step understandable without exposing private reasoning.

  • Identify source references.
  • Record constraints and approval state.
  • Keep action authority explicit.

Recover with evidence

Replay and recovery are architectural concerns. A system should be able to distinguish a failed action, an unreviewed proposal, and an incomplete evidence set before a recovery path is selected.

  • Separate replay from re-execution.
  • Preserve safe identifiers.
  • Escalate when evidence is insufficient.
